Tvrtka \u00a0RESPIRONICS pak je osnovana 1976. godine, na tr\u017ei\u0161tu je prisutna i danas, a kroz \u010ditavo razdoblje bavi se proizvodnjom medicinskih aparata i ure\u0111aja za pomo\u0107 pacijentima s pote\u0161ko\u0107ama u disanju. Respirator ili medicinski ventilator je ure\u0111aj koji mehani\u010dki upuhuje i ispuhuje zrak u i iz plu\u0107a pacijentima koji nisu u stanju (nesmetano) samostalno disati.<\/p>\n<p><a href=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_01.jpg\" rel=\"lightbox[3380]\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-large wp-image-3381\" src=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_01-1024x775.jpg\" alt=\"respirator_ lifecare_plv100_01\" width=\"1024\" height=\"775\" srcset=\"http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_01-1024x775.jpg 1024w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_01-300x227.jpg 300w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_01-768x581.jpg 768w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/a><\/p>\n<p>LIFECARE PLV-100 je elektri\u010dni prijenosni respirator dizajniran za dugoro\u010dnu upotrebu u ku\u0107noj njezi pacijenta. Mo\u017ee raditi na mre\u017eni napon 220V ili neki vanjski (baterijski) izvor istosmjernog napona od 12V. Tako\u0111er ima ugra\u0111ene i interne baterije ukupnog napona 12V koje omogu\u0107avaju respiratoru oko sat vremena autonomnog rada. Volumen i proto\u010dnost zraka iz respiratora LIFECARE PLV-100 mo\u017ee se precizno namjestiti u \u0161irokom rasponu tako da se mo\u017ee koristiti za sve pacijente (djecu i odrasle). \u00a0Sve funkcije respiratora kontrolirane su preko mikroprocesora.<\/p>\n<p><a href=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_02.jpg\" rel=\"lightbox[3380]\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-large wp-image-3382\" src=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_02-1024x925.jpg\" alt=\"respirator_ lifecare_plv100_02\" width=\"1024\" height=\"925\" srcset=\"http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_02-1024x925.jpg 1024w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_02-300x271.jpg 300w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_02-768x694.jpg 768w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/a><\/p>\n<p>Sa bo\u010dne strane ure\u0111aja montiran je nosa\u010d na koji se mogu spojiti dodatni vanjski ure\u0111aji za pobolj\u0161anje kvalitete vanjskog zraka koji se upuhuje pacijentu (npr. ovla\u017eiva\u010di zraka) ili priklju\u010dci za uvo\u0111enje plinova iz zasebnih spremnika (npr. spremnik kisika).<\/p>\n<p><a href=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_03.jpg\" rel=\"lightbox[3380]\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-large wp-image-3383\" src=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_03-1024x701.jpg\" alt=\"respirator_ lifecare_plv100_03\" width=\"1024\" height=\"701\" srcset=\"http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_03-1024x701.jpg 1024w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_03-300x205.jpg 300w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_03-768x526.jpg 768w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/a><\/p>\n<p>Sa zadnje strane isti\u010du se dva filtra za ulazni zrak, a izme\u0111u njih je senzor vanjskog atmosferskog tlaka kako bi se tome prilagodio izlazni tlak iz respiratora. Osim dva osigura\u010da i mre\u017ene priklju\u010dnice uo\u010dava se BNC konektor za povezivanje alarma respiratora sa vanjskim centralnim alarmnim sustavima bolnice.<\/p>\n<p>Kad se skinu filtri vidi se da je lijevo ulazni priklju\u010dak za uvla\u010denje vanjskog zraka, a desno je otvor za broja\u010d sati rada respiratora. Nakon odre\u0111enih sati rada respiratora potrebno je izvr\u0161iti i odre\u0111ene servise na respiratoru.<\/p>\n<p><a href=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_04.jpg\" rel=\"lightbox[3380]\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-large wp-image-3384\" src=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_04-1024x768.jpg\" alt=\"respirator_ lifecare_plv100_04\" width=\"1024\" height=\"768\" srcset=\"http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_04-1024x768.jpg 1024w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_04-300x225.jpg 300w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_04-768x576.jpg 768w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/a><\/p>\n<p>A sada se vratimo na prednju plo\u010du te kontrole i indikacije koje se nalaze na njoj. Postoji vi\u0161e parametara i pojmova vezanih uz proces i kontrolu disanja kod \u010dovjeka koji spadaju u specijalizirano podru\u010dje medicine i kao takvi nisu zanimljivi elektroni\u010darima. Ipak, kako bismo mogli bar donekle shvatiti kontrole koje nalazimo na elektri\u010dnim respiratorima lai\u010dki \u0107emo spomenuti tri najva\u017enija parametra kod disanja koji moraju biti podesivi kod svih respiratora:<\/p>\n<ul>\n<li>Respiratorni (tidalni) volumen &#8211; volumen zraka udahnut ili izdahnut u jednom ciklusu (respiriju). Kod na\u0161eg respiratora mo\u017ee se podesiti u rasponu od 0,05 &#8211; 3 l.<\/li>\n<li>Frekvencija (brzina) disanja &#8211; broj udisaja u minuti (BPM breaths per minute). Kod na\u0161eg respiratora mo\u017ee se podesiti u rasponu od 2 &#8211; 40 BPM.<\/li>\n<li>Inspiratorni: Ekspiratorni omjer (I:E) &#8211; omjer izme\u0111u vremena trajanja udisaja i izdisaja. Kod na\u0161eg respiratora ovaj omjer se regulira promjenom koli\u010dine zraka za udisaj (inspiratorni volumen) u rasponu od 10 do 120 l\/min.<\/li>\n<\/ul>\n<p><a href=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_05.jpg\" rel=\"lightbox[3380]\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-large wp-image-3385\" src=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_05-1024x768.jpg\" alt=\"respirator_ lifecare_plv100_05\" width=\"1024\" height=\"768\" srcset=\"http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_05-1024x768.jpg 1024w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_05-300x225.jpg 300w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_05-768x576.jpg 768w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/a><\/p>\n<p>Respirator LIFECARE PLV-100 ima tri na\u010dina rada:<\/p>\n<ul>\n<li>KONTROLLIERT &#8211; Kontrolirana ventilacija (potpuna kontrola disanja). U ovom modu respirator kroz cijelo vrijeme isporu\u010duje pode\u0161eni volumen zraka (0,05 &#8211; 3 l), propisanom frekvencijom (2 &#8211; 40 BPM) i u odre\u0111enom I:E omjeru (10 do 120 l\/min).<\/li>\n<li>ASSISTIERT\/KONTROL &#8211; Potpomognuta kontrola disanja. Uklju\u010deni su svi parametri kao i kod kontrolirane ventilacije s time da se u ovom modu prati napor (poku\u0161aj) udisanja kod pacijenta te se na osnovu toga onda prilago\u0111avaju parametri isporuke zraka pacijentu sukladno njegovim trenutnim potrebama. Tlak koji stvara pacijent prilikom (poku\u0161aja) udisanja slu\u017ei dakle kao okida\u010d praga osjetljivosti respiratora koji mo\u017ee biti pode\u0161en u rasponu -6 do +3 mbar. Na taj na\u010din je frekvencija i volumen isporu\u010denog zraka iz respiratora sinkroniziran sa di\u0161nim pokretima pacijenta.<\/li>\n<li>SIMV (MIT ZUSATZVENTIL) &#8211; Potpomognuta kontrola disanja isto kao i ASSISTIERT\/KONTROL s time da se ovdje iznad odre\u0111ene frekvencije disanja pacijentovi udisaji vi\u0161e ne podupiru ve\u0107 se otvara ulazni ventil i omogu\u0107ava pacijentu samostalan udah.<\/li>\n<\/ul>\n<p>Uz ova kratka poja\u0161njenja vi\u0161e manje su nam jasne sve ostale kontrole i pokaziva\u010di na sekciji MODE respiratora LIFECARE PLV-100:<\/p>\n<ul>\n<li>ATEMZUGVOLUMEN i VOLUMEN (0,05 &#8211; 3 l) &#8211; LCD i potenciometar za namje\u0161tanje respiratornog (tidalnog) volumena zraka<\/li>\n<li>ATEMFREQUENZ (2 &#8211; 40 min) &#8211; LCD i potenciometar za odre\u0111ivanje frekvencije (brzine) disanja (udaha u minuti). U modu KONTROLLIERT prikazuje se namje\u0161tena stalna brzina, dok se u preostala dva moda ra\u010duna srednja vrijednost svakih \u010detiri udaha.<\/li>\n<li>I:E VERHALTNIS &#8211; LCD za prikaz trenutnog omjera izme\u0111u vremena trajanja udaha i izdaha. Ovisi o namje\u0161tenim vrijednostima tidalnog volumena, frekvencije disanja i brzine protoka zraka.<\/li>\n<li>FLOW (10 do 120 l\/min) &#8211; LCD i potenciometar za namje\u0161tanje koli\u010dine protoka zraka isporu\u010denog u jednoj minuti (utje\u010de na I:E omjer). LCD prikazuje vr\u0161nu vrijednost protoka zraka kod udisaja. Ovaj LCD tako\u0111er slu\u017ei i kao voltmetar za mjerenje napona interne ili vanjske baterije.<\/li>\n<\/ul>\n<p>Sekcija STROMVERSORGUNG sadr\u017ei kontrole vezane uz napajanje respiratora. Ve\u0107 smo rekli da je napajanje mogu\u0107e preko mre\u017ee 220V, te vanjske ili interne baterije. Ako je priklju\u010deno i mre\u017eno napajanje i vanjska baterija respirator \u0107e koristiti mre\u017eno napajanje, a u slu\u010daju nestanka tog napajanja automatski \u0107e pre\u0107i na vanjsku bateriju. U slu\u010daju da vanjske baterije nema ili je potro\u0161ena, respirator mo\u017ee jo\u0161 oko sat vremena raditi sa ugra\u0111enim internim baterijama. Naponi vanjske i interne baterije mogu se mjeriti preko tipkala BATTERIESPANNUNG INTERN &#8211; EXTERN i na LCD pokaziva\u010du FLOW. Na prednjoj plo\u010di nalazi se mre\u017ena sklopka, osigura\u010di i priklju\u010dak za vanjsku bateriju.<\/p>\n<p>Sekcija DRUCK mbar sadr\u017ei kontrolu TRIGGERSCHWELLE (-6 do +3 mbar) koja slu\u017ei za namje\u0161tanje praga osjetljivosti kod pra\u0107enja disanja pacijenta u cilju automatske korekcije parametara respiratora u potpori disanja. Ovu funkciju opisali smo kod modova ASSISTIERT\/KONTROL i SIMV (MIT ZUSATZVENTIL). Manometar pokazuje tlak zraka u respiratornoj cijevi (-10 do +100 mbar). Tu su i tri priklju\u010dnice za cijevi respiratora preko kojih se pacijentu isporu\u010duje zrak i dobiva povratni tlak za okidanje kontrole TRIGGERSCHWELLE. Dva potenciometra (2-50 mbar i 10-100 mbar) slu\u017ee za postavljanje granica najni\u017eeg i najvi\u0161eg tlaka zraka na kojem \u0107e se oglasiti alarm.<\/p>\n<p>Respirator LIFECARE PLV-100 ima vi\u0161e zvu\u010dnih i svjetlosnih alarma koji upozoravaju na neke gre\u0161ke samog aparata ili dosegnute kriti\u010dne vrijednosti tlaka. Alarmne lampice upozoravaju na preniski respiratorni tlak, apneju (kratkotrajni prekidi disanja), pogre\u0161an odnos vremena udisaja izdisaja (I:E), pove\u0107an protok zraka pri udisaju, te niski napon interne ili vanjske baterije. Zvu\u010dni alarmi upozoravaju na preniski ili previsoki respiratorni tlak, apneju, niski napon interne ili vanjske baterije, pogre\u0161an polaritet spajanja vanjske baterije, gre\u0161ku u napajanju, nestanak mre\u017enog napona te na kvar respiratora.<\/p>\n<p><a href=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_06.jpg\" rel=\"lightbox[3380]\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter size-large wp-image-3386\" src=\"http:\/\/www.crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_06-1024x1014.jpg\" alt=\"respirator_ lifecare_plv100_06\" width=\"1024\" height=\"1014\" srcset=\"http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_06-1024x1014.jpg 1024w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_06-150x150.jpg 150w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_06-300x297.jpg 300w, http:\/\/crowave.com\/blog\/wp-content\/uploads\/2018\/04\/respirator_-lifecare_plv100_06-768x760.jpg 768w\" sizes=\"auto, (max-width: 1024px) 100vw, 1024px\" \/><\/a><\/p>\n<p>Prvo \u0161to se isti\u010de u ku\u0107i\u0161tu respiratora to je pumpa za pumpanje zraka. Izra\u0111ena je u obliku cilindra unutar kojeg se naprijed &#8211; natrag pomi\u010de klip i na taj na\u010din pumpa zrak. Princip je dakle potpuno isti kao i kod bilo koje ru\u010dne pumpe za zrak koju koristimo za zra\u010dnice bicikla ili automobila, s tom razlikom \u0161to ovdje klip precizno pomi\u010de mikrokontrolerom upravljan elektromotor. Vidimo da klip povezan sa osovinom elektromotora preko navojne \u0161ipke. Elektri\u010dna pumpa za zrak je izvedena posebno kvalitetno i precizno te garantira dugotrajan rad bez servisa. Samu pumpu pak kontrolira mikrokontroler povezan sa vanjskim senzorima i sa ugra\u0111enim posebnim programom koji izra\u010dunava optimalni protok zraka kroz plu\u0107a pacijenta u svim uvjetima promjene vanjskog tlaka ili promjene u samom na\u010dinu disanja pacijenta.
